Access To Care Program
The goal of PHK’s Access to Care program is to identify uninsured children and connect them to an appropriate and affordable source of quality health care services.
How the Access to Care program works:
- PHK
provides outreach materials and training to partnering school
sites.
- Schools conduct multi-level outreach.
- Uninsured children are referred to PHK.
- PHK provides comprehensive application assistance to connect children
to the most appropriate program for each child according to their
eligibility (FAMIS Plus, FAMIS, MCCP, LKHP, IPC, CHCN, and ANSHI)
- PHK follows up with families, schools and providers to help ensure children sustain enrollment.
Over 3,500 children were connected to a source of comprehensive health care services in 2003-2004 academic year.
