Purchasing CME Attendance Forms
The CME Office carries a very small supply of CME Activity Attendance Report forms ("half sheets") for emergency use. It is the responsibility of each department to order the attendance forms through Moore Wallace. The form number is CAT #97917.
Please keep an adequate supply of CME Activity Attendance Report forms (half sheets) on hand. Physicians are required to submit these forms to obtain educational credit for re-credentialing requirements.
