Home Health Needs
To insure that your post-hospital care meets your needs, discharge planning actually begins at admission. Your physician and nurse will help you coordinate care or services needed after you leave the hospital. You or your family may also call the Care Management Department at 703-504-3580 to discuss various post-hospitalization options and services.
Inova Health System offers home health care, home health equipment, private nursing care, nursing home or intermediate care, assisted living and visiting nurses. You may wish to contact Inova VNA Home Health, the Home Health Equipment Company, or our two long-term care facilities, Inova Cameron Glen Care Center and Inova Commonwealth Care Center, directly to learn more about these services. The Care Management Department has brochures and other information. These services and equipment also are available from other providers or suppliers in the community, and the Care Management Department will provide you with this information upon request.
