Teen Volunteer Application

Please complete the following steps:

1. Print out the teen application form (.pdf) and complete it.
   
   
2. Print out two personal reference request forms (.pdf). Ask two people who are not family members or members of your household to serve as your references and to complete the forms.
   
   
3. Contact the Volunteer Services office at 703-776-3104 or e-mail ifh.volunteers@inova.org to sign up for one of the mandatory information meetings prior to the start of each session. You MUST be accompanied by a parent at the information meeting.
   
   
4. Bring completed application and reference forms to the information meeting.
