- Referral to the Inova Transplant Center can be initiated
via phone call or fax
from:
- Nephrologist
- Dialysis Unit
- Patient
- Insurance Company
- referral secretary takes demographic and insurance information
- financial approval requested
- medical records and copies of insurance cards requested
- patient scheduled for transplant education class
- patient scheduled for initial team appointments after financial approval obtained
